This article sheds more light on the term "foot orthotics" - what they are, how they work, the different types of orthotics available and which common complaints can be treated with orthotics. Lower limb findings associated with hypermobility are pes planus, out-toeing gait, over-pronated feet (secondary to ankle hypermobility) and genu recurvatum (hyperextensibility of the knee joint), all of which may be improved by the use of custom-moulded hard insoles aimed at supporting the longitudinal foot arch and stabilising the ankle. This is the appearance of pes planus, or flatfoot, which occurs in up to 20% of adults with no other abnormalities. There are many contributors to pes planus deformity, including hindfoot valgus shown by increased talocalcaneal angle to greater than 45 degrees, midfoot sag or lisfranc ligament injury, and forefoot pronation (seen as overlapping metatarsals in the images above). Perhaps the best exercise for keeping the arch healthy is the "towel scrunch." With time and decreased demand on our feet, the small muscles become weaker. Another way to increase arch strength is toe raises on a stair or raised board. I quite like the term Pes Planus.

Shoes are practical and good, but if your shoes do not provide adequate arch support, they may contribute to foot problems. To improve your arches and benefit overall foot health, try to go barefoot as much as possible. If you have access to a beach, you can give you arches a great workout by walking barefoot in the sand one or two times each week. According to Aadil Palkhivala of "Yoga Journal," yoga offers many benefits for improved foot arches. Slide the feet out until they are a bit wider than your hips, keeping the tops of your feet against the floor. Try to sit back all the way between your feet, or place a rolled towel under your buttocks if you can't make it all the way down. If you experience cramping, you can also massage your feet while in the pose. A ballerina's feet may be the epitome of beautiful arches.

Your body weight is evenly distributed between your two feet, but you may experience pain and discomfort when moving because of the added strain on your joints as your overpronate. You will experience a sharp pain or soreness in the area of inflammation - often indicative of joint, tendon or muscle damage or pressure - especially when running.

In cases of PTTD that have progressed substantially or have failed to improve with non-surgical treatment, surgery may be required. Subtalar arthroereisis involves inserting an implant into the sinus tarsi through a small incision to block excessive subtalar joint pronation and to elevate the talus. Although Timothy C. Fitzgibbons, MD, and his colleagues at Creighton University/University of Nebraska Medical Center have been critical of use of the arthroereisis implant in the asymptomatic flexible flatfoot patient—especially in patients younger than age 10 years—and have not recommended it in older symptomatic children, they were willing to test its effectiveness in a small study. Dr. Mosca also believes that the number and types of complications associated with subtalar arthroereisis, including inappropriate indi-cations, are underrepresented in the literature. Try to give the feet as much rest as possible.
